Policies
v Swimwear returns for any reason are prohibited under state health codes. You should thoroughly soak your swimsuit in fresh water before entering any chemically treated swimming pool, and then thoroughly rinse it after getting out. This will help to lengthen its fabric’s lifetime. Pool chemicals do break down fabrics, however, and we are unable to accept returns of swimwear due to chlorine damage.
v Your bras may be machine washed and then air dried. Bra defects are covered under a six month warranty, so should you experience any problem other than normal wear and tear within that six month period, you may return them. Examples of defects covered under your bra warranty are strap breakage, problems with the hooks and/or eyes, etc.
v Your silicone breast prosthesis should be cleaned daily with mild soap or breast form cleanser and then air dried. The breast form is covered under a two-year warranty (with the exception of silicone swim prosthesis) for manufacturer defects only. This includes silicone separation, splits or tears, and leaking around the seam.
v Your foam or fiberfill breast form and/or your silicone swim prosthesis are covered under a six month warranty for the same type of defects listed above. You should clean your swim-form with mild soap and air dry daily (or with each wearing). Your foam breast form should be hand washed or machine washed on delicate cycle and air dried at least every other day. To clean your fiberfill breast form, you should remove the fiberfill “stuffing” and machine wash the cover. The fiberfill may be replaced as needed.

v If we will be billing an insurance company for you, we will file your primary insurance first, then any secondary insurer. You will receive statements from these insurers stating the amounts they paid and any amounts owed by you. PLEASE DO NOT REMIT ANY PAYMENT TO US UNLESS YOU RECEIVE A BILL FROM US. Your claim may be in the process of being appealed or reprocessed (in the case of a denial) or your secondary carrier may not have remitted their portion yet.
v We will periodically send out mailings about new products and special events. If you do not wish to receive these mailings, let us know so that we can remove your address from our mailing list. If your address changes, we also need to know so that we can update our files.

v You are also entitled to request to inspect or obtain copies of your medical files. To do so, you must fill out a Patient Request to Protected Health Information Form. You can pick this form up at our office or request that one be mailed to you.
